At Physiotherapy Associates we are committed to safeguarding the privacy of our clients and visitors to our website. This policy lets you see how personal information is treated. The policy applies to all information collected via Telephone, Social Media sites, Website, Forms.

What information we collect
We may collect, store and use the following information: Information that you provide over the telephone or website when making an enquiry or booking an appointment ( name, telephone number, reason for enquiry). Information that you provide at your first appointment/filling consent form (name, address, telephone number, e-mail, DOB, GP details, occupation, hobbies, medical history) Information via other communication means- Facebook Information (Bank/ card details) when purchasing online or pre-paying for appointment.
 
How we use this Information
We may use your information to
  • Liaise with GP/ Consultant/ Insurance Company if required
  • Administration purposes- sending of invoices, and collection of payments
  • Communicate with you re upcoming events/classes which might be of interest to you
  • Send you any other marketing communications (you can inform us at any time if these are not required).
  • We will not at any time give your personal information to any third party without your prior consent.
Disclosing Personal Information
We may disclose your information where necessary to
  • Other therapists, suppliers, or employees
  • Other governing bodies (when required by law to do so)
  • Our operating system TM2 which is hosted by Blue Zinc
  •  In connection with any legal proceedings, in order to establish, exercise or defend our legal rights (including providing information to others for the purposes of fraud prevention and reducing credit risk).
  • To the purchaser (or prospective purchaser) of any business or asset we are selling.
Security of your Data
  • We will take all reasonable technical and organisational precautions to prevent the loss, misuse or alteration of your personal information
  • All paper data is stored in a locked filing cabinet
  • All electronic data is stored on password protected servers
  • We do not store Credit or Debit card details
  • You acknowledge that transmission of information via Internet may be insecure- we cannot guarantee the security of such
How Long Do we hold this information
  • We are obliged by our governing body (CSP) to hold patient records for 8 years following discharge.
  • For patients under the age of 18 years records are held until their 25th birthday
  • At these times, paper notes are shredded and electronic notes deleted
Amendments
This privacy statement may be amended/updated from time to time- any updates will be added to our website
 
Your rights
  • You may instruct us to provide you with any information we hold about you.
